REGISTRATION CONFIRMATION
For express enrollment and confirmation, REGISTER ONLINE today! Our online secure viewer protects your information and expedites the registration process. If you wish to register by mail or fax, please download and complete the application form and return to the Beasley office (P.O. BOX 5759, Fort Wayne IN 46895 or Fax: 260-496-8116). Once your application is received and processed, you will receive a confirmation acceptance via email.
FAMILY DISCOUNTS AND MULTIPLE CAMP DISCOUNTS
* Campers from the same immediate family will receive a discount of $10 for each additional sibling.
* Any camper who registers for more than one camp will receive $10 off each additional registration.
* Any Camper attending 4 weeks of summer camps with the Beasley National Soccer School will receive autographed DaMarcus Beasley customized NIKE Shoes that he wears during his professional games.
TEAM & GROUP DISCOUNTS
There must be at least 10 or more campers in the group/team and all applications must be received at the same time in the same envelope to receive a team discount. $10 discount may be deducted off each application.
PAYMENT POLICY
Full tuition is due with completed application. Registration payments may be made by personal check, money order, MasterCard or Visa. When paying by credit card, the full amount will be charged. Payment plans may be available, please call 260-469-3024
DAY CAMP REFUND & CANCELLATION POLICY
To receive a full refund, a cancellation must be received in writing by mail, fax, or email no later then one week prior to the start of camp. Should you have to cancel less than one week prior to the camp start date, a refund will be provided minus a $25 Processing fee. No refunds or credits will be given for no-shows or expulsions from camp.
